Rural Health Transformation Funding Consulting
Funding Application Due Date: 04/17/26
Opportunity Description
The Tennessee Department of Health seeks input from qualified evaluation consultants to support the design and implementation of a rigorous, mixed-methods evaluation for the Rural Health Transformation Fund, a multi-year initiative funded by the Centers for Medicare and Medicaid Services. The program aims to improve health outcomes across all 89 counties and 6 metro regions by ensuring that Tennesseans can access better care closer to home.
The anticipated evaluation will assess investments in rural health innovation, access to care, workforce development, and patient-centered care models. The work will focus on measuring program effectiveness, sustainability, and impact across diverse communities, with an emphasis on evidence-based interventions, system coordination, and improved health outcomes in rural and underserved areas.
